Directors Annual Agreement & CIO

To remain a Board member, each ID8 Board of Directors Member is required to execute this annual agreement at the beginning of each calendar year and will only be a voting Member after this form is completed. Agreement terms are derived from our Bylaws that specify a number of Director provisions including but not limited to:

• • Director appointment, meeting notice, voting and other rights.
  • Director indemnification and insurance provisions.
  • Director Conflict of Interest Policy.
  • Director annual gift requirement (you may give as little or as much as you desire including naming ID8 in Wills and Trusts. Gifts will not be disclosed and are only required so to facilitate a 100% Director giving percentage that is a factor in many grant assessments).
